Job Description

Join the Lincolnshire team as a Leaving Care Wellbeing Worker, where you’ll play a vital role in supporting care-experienced young people aged 16–25 as they transition into independent adulthood. This is a relationship-focused role where you’ll offer practical guidance, emotional support, and advocacy to help young people build resilience, confidence, and life skills. About the role As a leaving care worker you will be focused on Wellbeing of CEYP in the Leaving Care team, you’ll be a vital point of contact for care-experienced young people aged 16–25. You’ll provide tailored support, guidance, and advocacy to support their transition into independent adulthood with confidence and resilience, with a focus on wellbeing. This is a relationship-based role where your ability to connect, empower, and advocate for young people will make a lasting difference. You’ll work with: • Young people still in care • Care leavers aged 18–21 • Unaccompanied asylum-seeking young people • Older care-experienced individuals aged 21–25 As a LC Wellbeing Worker, you will: • Provide holistic one-to-one support. • Build trusted relationships with care-experienced young people. • Offer tailored wellbeing support that addresses emotional, mental, and physical health needs. • Design and deliver wellbeing activities that promote self-esteem, resilience, and positive coping strategies. • Promote mental health awareness, supporting young people in understanding and managing their mental health, reducing stigma, and encouraging access to appropriate services such as CAMHS, adult mental health teams, or counselling. • Support transitions to independence, helping young people develop life skills, routines, and confidence as they move into independent living, education, training, or employment. • Advocate for young people’s needs, acting as a voice for care leavers in multi-agency meetings, ensuring their views and wellbeing needs are heard and respected. • Build community connections, linking young people with local services, peer support groups, and community resources that enhance their wellbeing and reduce isolation. • Monitor and record progress, maintaining accurate, confidential records of support provided, outcomes achieved, and any safeguarding concerns, using appropriate case management systems and SMART targets. • Safeguard and respond to risk, identifying and respond to safeguarding concerns in line with policy and procedure, working closely with social care and health professionals. • Contribute to service development, sharing insights and feedback to help shape and improve the wellbeing offer for care-experienced young people, ensuring it remains responsive and inclusive. • Travel locally and nationally for face-to-face visits.
