About the Role
We are recruiting a Support Worker to join a community-based mental health service in the Telford area. You will work closely with adults who require support to maintain their wellbeing, build confidence, manage their tenancy and develop independence.
Your role is centred around forming warm, trusting relationships and promoting positive mental health and emotional wellbeing. You will support service users both within their homes and the wider community, offering practical, emotional and recovery-focused assistance.
Key Responsibilities

Build and maintain warm, trusting relationships to help individuals express their needs and concerns
*
Promote dignity, privacy, independence and emotional wellbeing
*
Follow Individual Support Plans and respond flexibly to changing needs
*
Support service users with tenancy management, home safety, hygiene and comfort
*
Act as a positive role model in the home and wider community
*
Encourage engagement with social, leisure, education and wellbeing activities
*
Support with budgeting and financial management
*
Assist individuals in developing daily living skills and coping abilities
*
Monitor and report changes in mental health or wellbeing
*
Support individuals to access healthcare services and maintain appointments
*
Work within risk management guidelines and escalate concerns appropriately
*
Maintain accurate records and contribute to reviews and support plans
*
Carry out domestic tasks and personal care support where required
*
Support people with mobility needs, including wheelchair use or hoist assistance
Person Specification
We are looking for someone who can demonstrate:

Experience in supported accommodation or mental health support (desirable)

NVQ Level 2+ in Health & Social Care (desirable)
