Are you a driven and compassionate Social Worker passionate about supporting adults with learning disabilities through significant life transitions? We're seeking a dedicated professional to join our Transitions Team in Doncaster.
About the Role:
Work directly with adults with learning disabilities, supporting them to navigate complex transitions such as moving to more independent living, changing support providers, or accessing new services.
Develop and implement person-centered transition plans, ensuring seamless and positive outcomes.
Collaborate closely with families, care providers, and other professionals to coordinate support and advocate for individual needs.
Conduct thorough assessments and provide ongoing support and guidance to individuals during their transition journey.
Contribute to a supportive and dynamic team committed to improving the lives of adults with learning disabilities.
Requirements:
Registered with Social Care England.
Minimum 3 years' post-qualification experience as a Social Worker.
Experience working with adults with learning disabilities, ideally within a transition-focused setting.
Excellent communication, problem-solving, and case management skills.
A proactive and solution-oriented approach to supporting individuals through change.
Commitment to person-centered care and ethical practice.
