Tripod Partners are looking for a Band 6 Nurse to support the Continuing Healthcare (CHC) team in Mid and South Essex. This role is open to General Nurses or Specialists with experience in CHC assessments and reviews. The ideal candidate will be able to carry out face-to-face assessments in the local area.
Key Responsibilities:

Conduct CHC assessments, reviews, and decision support tools in line with national guidance.

Liaise with patients, families, and multidisciplinary teams to support eligibility decisions.
*
Document clinical findings accurately and contribute to robust care planning.
*
Support the CHC team in delivering high-quality, compliant service delivery.
Position Details:
Location: Mid and South Essex
Duration: 3 months
Hours: 2.6 WTE (Full-time and part-time hours available)
Pay Rate: £26.00 per hour
Work Pattern: Monday to Friday
Requirements:

NMC registered Nurse (essential).
*
Previous experience in Continuing Healthcare (essential).
*
Based in or able to travel across Mid and South Essex for face-to-face assessments (desirable).
*
Strong assessment and communication skills.
*
Right to work in the UK
