Lead & Transform Complex Care in Devon! Full-Time/Part-Time Options Available. £50,000-£55,000 per annum.

Role
Role & Responsibilities
You will conduct in-depth health assessments, develop and implement bespoke care plans, and coordinate care with a multi-disciplinary team. You'll empower patients and their families, monitor outcomes, and drive continuous improvement in care delivery. This leadership role offers the chance to make a tangible difference in complex care.
Main tasks include
Conducting in-depth health assessments.
Developing and implementing bespoke care plans.
Coordinating care with GPs, therapists, and social workers.
Ensuring the safety and effectiveness of care packages.
Contributing to local business growth and commissioning initiatives.
Empowering patients and their families.
Monitoring outcomes and driving continuous improvement.
Requirements
NMC Registered Nurse: Active PIN in Learning Disabilities Nursing or Children's Nursing.
Proven experience in complex care or related nursing roles.
Proactive, compassionate, highly organised, and a natural leader.
Excellent communication and interpersonal skills.
Strong analytical and problem-solving abilities.
Ability to work independently and as part of a team.
Commitment to providing high-quality patient care.
Experience with care planning and coordination.
Knowledge of relevant legislation and guidelines.
