Job Title: Social Worker – Young Adults Team (16–25), All Age Disability Service
Location: County Hall, Worcester
Working Hours: 37 hours per week

Pay rate for this role will be 32.67LTD per hour or £25 PAYE per hour
Job Summary We are seeking a qualified and experienced Social Worker to join Worcestershire’s All Age Disability (AAD) Service, working specifically in the Young Adults Team supporting individuals aged 16 to 25. This role requires specialist knowledge across both Adult and Children’s Services, with a strong focus on supporting young people with Special Educational Needs (SEN) and disabilities as they transition into adulthood.
Key Responsibilities
Work with young people aged 16–25 with learning disabilities, physical disabilities, or complex health needs.
Support transition planning in key areas such as education, employment, independent living, community inclusion, and health.
Engage in multi-agency working with Children’s Services, Adult Services, SEND, health professionals, schools, and colleges.
Apply both children’s and adult social care legislation effectively in practice.
Conduct assessments and develop care/support plans using Liquidlogic (children and adults systems).
Contribute to positive outcomes for young people preparing for adulthood.
Ensure regular case supervision and maintain accurate records.
Travel across Worcestershire and occasionally nationally for placement visits.Required Qualifications & Experience
Qualified Social Worker with relevant professional registration (e.g., Social Work England).
Experience working in transitions between Children’s and Adult Services.
Sound knowledge of legislation relevant to children and adult social care.
Skilled in multi-agency collaboration and person-centred planning.
Proficiency in using Liquidlogic or similar case management systems.
A valid driving licence and access to a vehicle are essential.Preferred Qualifications
Experience working within an All Age Disability framework.
Additional qualifications in SEN, autism, or related fields.What We Offer
