Role Overview:
Sanctuary Personnel is currently recruiting on behalf of Swansea Council for a qualified Social Worker to join the Transitions Team within Adult Services. This post sits within the Community Learning Disability Team and is offered on an initial three-month temporary contract.
The role focuses on supporting young people with learning disabilities as they transition from Children’s Services into Adult Social Care – a critical period that requires a sensitive, coordinated, and person-centred approach.

Key Responsibilities:
Carrying out needs-led assessments and reviews in line with the Social Services and Well-being (Wales) Act 2014
Developing person-centred care and support plans in partnership with individuals and their families/carers
Liaising with Children’s Services to ensure smooth and supported transitions
Attending safeguarding and welfare visits as required
Maintaining accurate, up-to-date case records on the WCCIS system (experience preferred)
Working Arrangements:
Hybrid working model: part-home working, part-office based (Guildhall, Swansea), and face-to-face client visits
Flexibility required to travel locally for community visits
Candidate Requirements:
Degree in Social Work and registered with Social Care Wales
Experience working with adults or young people with learning disabilities, preferably in a transitions setting
Knowledge of the legislative framework relevant to adult social care in Wales
Familiarity with the WCCIS system is advantageous
Enhanced DBS on the Update Service (or willingness to obtain)
