About the role We are looking for two Transition Support workers who will be based at our school in Nottingham City Centre, Sutherland House School. This is an exciting new project launching in April 2025 to support autistic young people and adults across the East Midlands. We currently have two vacant posts, one role focusing on children and young people and one role focusing on adults. Both roles will focus on supporting young people and adults transition into adulthood to secure meaningful long-term outcomes. The purpose of the role is to provide a consistent presence in the life of young people aged 14-25 years of age to enable a supportive transition into adulthood. The role will enable long-term positive outcomes, provide signposting for support available including housing and benefits and assisting in employment or further education opportunities for autistic young people and adults including supporting access to work. What will you do Our Transition Support Workers will provide one-to-one support to autistic individuals. This will include both face-to-face and online meetings and the securing of long-term outcomes for those who access the Transition Support Package. Purpose of the Post: * To be a consistent presence in the life of a young person into adulthood (aged 14-25 years) to support a meaningful and positive transition into adulthood. * To work to support young people to identify ambitions for the future. * To support young people from the age of 14 years to enable fulfilment of ambitions into adulthood. * To work with internal and external partners to identify and work with to achieve outcomes e.g. accommodation, benefits, employment, education, training, apprenticeships. * To advocate on behalf of young people internally and externally. * To liaise with the external evaluation team to in design and implementation of the project to provide a meaningful evaluation report at the end of the two-year project. * To support autistic young people and adults leaving school with their transition to their next setting * Assist in the development and implementation of transition plans for pupils and adults * Assist and support in MAP meetings and EHCP meetings to create and develop action plans. * Work collaboratively with teachers, families, and external agencies to ensure positive transitions for young people into their next setting. * Provide emotional and practical support to pupils during their transition into adulthood * Maintain accurate and up-to-date records including communication logs. What we are looking for: * Consistent and proactive support for the autistic young people and adults we support * Interpersonal skills to work with internal and external partners including accommodation, benefits and employment services * Effective communication abilities * Empathy and understanding * Commitment to continuing professional development * Willing to support and promote Autism East Midlands at local events * Ability to maintain and develop recording systems * Ability to work with an external evaluation team to design and implement the project leading to a formal evaluation of outcomes linked to the role Most importantly... We want a consistent advocate to enable opportunities for autistic young people and adults to secure employment and education leading to positive outcomes. The post-holder should be someone that is passionate about helping autistic individuals navigate the barriers that young peoples may face when transitioning from children’s services to adult services.
