Admissions & Transition Coordinator Contract type: Full Time, Permanent Salary: up to £25,000 Hours: Mon-Thurs 8.15am-4.30pm, Fri 8.15am-3.15pm Start date: 29th September 2025 Your new role Are you passionate about supporting young people through key educational transitions? We are looking for a dedicated Admissions & Transition Coordinator to join our team and play a vital role in shaping the journey of new pupils entering our school who have a range of Social, Emotional and Mental Health needs. Key Responsibilities: ● Coordinate the assessment stage of pupil admissions and oversee the baselining of new pupils. ● Establish supportive relationships and trust with prospective parents/carers and new pupils through the Admissions and Transition process; including home visits and school tours. ● Review referral documents and conduct qualitative assessments of need. ● Liaise with current provisions, families/carers, and external support networks to develop effective support plans. ● Prepare detailed assessment reports to support placement decisions. ● Coordinate the transition of accepted pupils into their school placement. ● Present initial assessments at the HONOSCA panel to inform therapeutic support planning. ● Collaborate with the SENCo to review and refine pupils' provision maps. ● Develop and oversee safety and wellbeing support plans using a child-centred approach. ● Support new pupils in achieving full-time attendance through structured transition plans. What We're Looking For: ● A compassionate, organised, and proactive individual with experience in education, social care, or a related field. ● Strong communication and interpersonal skills to work effectively with families, professionals, and young people. ● A commitment to inclusive practice and supporting pupils with social, emotional, and mental health needs. Required Experience and Qualifications: ● At least two years' experience working with families and vulnerable Children/Young people ● GCSE in English and Maths (A*-C) or Adult Literacy & Numeracy Level 2 equivalent ● A-levels (desirable) or Further Professional Qualifications particularly in a Psychology/Sociology/Social Care/Youth work related field (desirable) ● DSL trained (desirable) Your new school Quorn Hall is an inclusive and forward thinking school that supports Children and Young People aged 6 -17 with Social, Emotional and Mental Health needs.
