Your main duties will include:
Supporting and advocating for adults with complex health and care needs through the Continuing Healthcare (CHC) pathway.
Working collaboratively with multidisciplinary professionals, service users, and families to assess needs and develop effective care plans.
Contributing to CHC assessments, reviews, and decision-making processes to ensure individuals receive appropriate funding and support.
Promoting independence, dignity, and person-centred care for adults with long-term health conditions and complex needs.
