Adult Social Worker - 0-25 Transforming Care Team
Location: Hertfordshire (Countywide)
Pay: £32 per hour

About the Role:
We are seeking an experienced Adult Social Worker to join the 0-25 Together Service within the Transforming Care Team. This role focuses on supporting young adults aged 18-25 with complex mental health needs who are at risk of hospital admission.
You will work both remotely and from office locations across the county, so a full driving licence and access to a car are essential. Due to the nature of the work, we require a female social worker to support individuals living with trauma who may be triggered by male workers.
Key Responsibilities:
Complete Care Act and Capacity assessments.
Undertake Community DoLs applications and prepare reports for court.
Support transitions in and out of hospital admission.
Carry out risk assessments and work within a multi-agency team including mental health services, learning disability teams, schools, colleges, and providers.
Apply knowledge of CETR, DSR, and the Mental Health Act.What We're Looking For:
Qualified Adult Social Worker with current registration.
Experience in adult safeguarding and working with learning disabilities and transition cases.
Strong understanding of complex mental health needs and risk management.
