Locum Adults Social Worker – Hospital Discharge to Assess (HDTA)
Manchester City Council is seeking an experienced Locum Adults Social Worker to join the Hospital Discharge to Assess (HDTA) Team based at Manchester Royal Infirmary. This role is integral to ensuring safe and timely discharges from hospital settings, working closely with NHS colleagues and multidisciplinary teams.
Key Responsibilities:
Undertake Care Act 2014 assessments
Complete Mental Capacity Assessments (MCA) and Best Interest decisions
Engage in safeguarding enquiries and multi-agency working
Work within hospital settings and community hubs to facilitate person-centred discharge planning
Maintain accurate case recordings and adhere to statutory responsibilitiesRequirements:
Qualified Social Worker (Degree/DipSW/CQSW)
Social Work England registration
Minimum 3 years’ post-qualifying experience in Adults Social Care
Previous HDTA or hospital discharge experience is essential
Strong working knowledge of the Care Act, MCA and safeguarding frameworks
Able to provide 3 years of professional references (employment gaps must be accounted for)Working Arrangements:
Based 2–3 days/week at MRI (rota-based) remainder remote working
Weekend cover required on a 1-in-6 basis (Saturday at MRI, Sunday at Gorton Hub)
