Are you a passionate and experienced Social Worker looking to make a real difference in the lives of adults with learning disabilities? We're seeking a dedicated and empathetic individual to join our highly skilled team in Leeds.
About the Role:
Provide high-quality support and care to adults with learning disabilities, promoting their independence and well-being.
Collaborate with individuals, their families, and other professionals to develop and implement person-centered care plans.
Conduct thorough assessments to identify needs and support individuals in achieving their goals and aspirations.
Advocate for individuals' rights and access to appropriate services and support.
Contribute to a positive and supportive team environment focused on client-centered care.
Requirements:
Registered with Social Care England.
Minimum 3 years' post-qualification experience as a Social Worker.
Experience working with adults with learning disabilities is highly desirable.
Excellent communication, interpersonal, and case management skills.
Commitment to person-centered care and ethical practice.
