What you will do: - Conduct Care Act assessments, reviews, and support planning for adults with care and support needs, ensuring each individual's requirements are met. - Take the lead in Section 42 safeguarding enquiries and conduct Mental Capacity Act assessments, maintaining compliance and safety at all times. - Manage a dynamic caseload of 16 to 20 individuals, effectively prioritising urgent visits and risk planning with a strategic mindset. - Empower individuals with a strengths-based, person-centred approach, promoting choice, self-determination, and control. - Work collaboratively with health, housing, and voluntary teams, engaging with multi-disciplinary colleagues to provide comprehensive care solutions. - Adhere to internal key performance indicators, maintaining accurate case records and fulfilling statutory reporting in line with legal and organisational standards. - Participate actively in duty rotas, multi-agency meetings, and mental health discharge meetings, contributing to a cohesive team effort.
