What you will do:
Work collaboratively with the team manager to support the locality team.
Engage with individuals over 18 in the community to complete Care Act Assessments.
Incorporate the Mental Capacity Act and DOLS while assisting with court work.
Support with Continuing Health Care Assessments to ensure comprehensive care provision.
Be proactive in identifying needs and developing care plans in collaboration with other professionals.
