As an Assistant Practitioner, your responsibilities will include:
Supporting the assessment, planning, and evaluation of personalised healthcare for service users
Collaborating with a multidisciplinary team to deliver high-quality care
Maintaining high standards of care delivery in line with national guidelines and company policies
Communicating effectively with colleagues and providing accurate reports on patient conditions
Promoting choice and service user participation in clinical practice
Upholding a safe working environment and maintaining confidentiality

About You
The ideal candidate for the Assistant Practitioner position will have:
A Level 5 Foundation Degree in Assistant Practitioner (Healthcare)
Experience in healthcare, ideally within a specialist service like Neuro/Palliative care
Knowledge of relevant legislation impacting current practice
Strong communication and interpersonal skills
Ability to work effectively within a multidisciplinary team
Compassion and responsiveness to individuals in distress
