Job Overview
As a CHC Assessor, your primary role will be to evaluate patients' needs to determine eligibility for Continuing Healthcare funding. You will work closely with multidisciplinary teams to deliver high-quality assessments that directly impact patient outcomes. Key Responsibilities
Conduct comprehensive assessments of patients’ health and social care needs
Review medical records and collaborate with healthcare professionals

Ensure compliance with NHS policies and legal standards
Maintain accurate records and report findings in a timely mannerCandidate Requirements
Current registration as a CHC Nurse (or equivalent)
Proven experience in conducting CHC assessments

Strong communication and interpersonal skills
Ability to work independently and within multidisciplinary teams
