Remote Band 6 PHB Assessor
Location: Remote (UK-based role)
Hours: 37.5 per week
Pay Rate: £26.28 per hour
Contract Type: Ongoing assignment

In this role, you will carry out detailed assessments for individuals who may be eligible for an NHS Personal Health Budget (PHB). Your expertise will help ensure people receive the financial support they need to manage their health and wellbeing more independently, enabling them to live more fulfilling lives.
Key Responsibilities:
Complete holistic assessments to determine eligibility for PHB funding.
Work in partnership with service users, families, and healthcare professionals to identify individual needs and outcomes.
Prepare clear, evidence-based recommendations and documentation in line with NHS guidance.
Support the development of personalised care and support plans.
Maintain accurate records and ensure all assessments comply with governance and professional standards.
Requirements:
Registered professional (Nurse, AHP, or equivalent qualification).
Experience in assessments, care planning, or commissioning within the NHS.
Knowledge of PHB processes, NHS Continuing Healthcare, or similar frameworks.
Excellent communication, report-writing, and time management skills.
Ability to work independently in a remote environment.
