Job Description
Calderdale Council is seeking experienced Qualified Social Workers (QSWs) to join our Hospital Discharge Team and Post-Discharge Assessment Team.
This is an exciting opportunity to work in a fast-paced, supportive environment where you will play a key role in facilitating safe and timely hospital discharges, and ensuring continuity of care within the community.
Key Responsibilities
Undertake hospital discharge assessments for patients at Huddersfield Royal Infirmary and Calderdale Royal Hospital.
Work collaboratively with health professionals, patients, and families to plan and coordinate safe discharge arrangements.
Where required, provide cover for the Post-Discharge Assessment Team, assessing and reviewing individuals discharged into community settings - whether returning home or moving into care placements.
Ensure compliance with the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation and policy frameworks.
Maintain accurate and timely case records and documentation.
Requirements
Qualified Social Worker (Degree in Social Work, CQSW, DipSW or equivalent).
Current registration with Social Work England.
Experience within hospital discharge, adults, or community social work teams.
Strong assessment, decision-making, and communication skills.
Ability to work effectively within a multi-disciplinary team.
Enhanced DBS check required.
