Job Description
Job Title: Care Coordinator
Salary: Up to £32,000 PA DOE
Location: South London
This is an excellent opportunity for an experienced care coordinator to join a leading independent home care provider within their South London branch and support the delivery of personalised, high-quality self-funded care.
Our client deliver up to 2000 hours of care per week, and have recently been inspected as Outstanding by CQC.
Requirements
Ideally NVQ Level 2 or 3
Previous experience within a scheduling role
Knowledge of CQC compliance is essential
Good working knowledge of IT Systems
Ideally experienced in conducting care assessments, care reviews and being able to motivate care staff communicate effectively and efficiently via phone, email and in-person meeting
Responsibilities
Day to day management of schedules around caregivers and clients.
Working with clients and their families
Ensure care delivered is compliant with all relevant legislation
Undertake assessments and reviews
Undertake supervisions and direct observations
Improving and maintaining the continuity of care
Remuneration and Benefits:
Up to £30,000 PA DOE
