Job Overview: As a Care Coordinator/Deputy Care Manager, you will work alongside the Care Manager to oversee the delivery of person-centred care services for our clients and develop our client base. You will coordinate needs assessment and care plans, build a new service from the ground up and manage a team of carers, and ensure the highest standards of care are consistently met. Key Responsibilities: * Assist the Care Manager in the day-to-day management of care services. * Develop and review individual care plans, ensuring they reflect the needs and preferences of clients. * Coordinate and monitor the delivery of care services, ensuring they meet regulatory and quality standards. * Supervise and support a team of care staff, providing guidance, training, and performance management. * Conduct client assessments and ensure appropriate care is provided in line with personalised care plans. * Act as a point of contact for families, external agencies, and healthcare professionals. * Ensure effective communication within the team and with other relevant parties. * Maintain accurate and up-to-date client records and documentation in line with company policies and procedures. * Promote a positive and supportive environment for clients and staff. Skills and Experience: * Previous experience in a care coordination or deputy care management role, or a similar field. * Strong understanding of person-centred care principles and regulatory standards. * Excellent communication skills, both written and verbal. * Experience in managing and supporting a team of carers. * Good organisational and time-management skills with the ability to prioritise tasks effectively. * A compassionate and empathetic approach to working with clients. * NVQ Level 3 in Health and Social Care (or equivalent) is desirable; Level 5 is a bonus. * A commitment to ongoing professional development.
