We are currently recruiting a Care Coordinator to support children and adults with complex physical disabilities and life-limiting conditions across West London. This is a fantastic opportunity to join a compassionate, nurse-led team delivering high-quality care in clients’ homes.
What you’ll do as a Care Coordinator:
Coordinate and oversee the mobilisation of new complex care packages
Set up care plans, complete risk assessments, and ensure regulatory compliance
Liaise with clients, families, healthcare professionals, and internal teams
Ensure timely and seamless delivery of care services
Maintain strong working relationships across the organisation

What we’re looking for:
Previous experience in coordinating care packages (complex care preferred)

Confident in handling data and making informed decisions
