Location: Maidstone, Kent
Salary: £28,000 per annum

Leaders in Care is proud to be recruiting on behalf of a respected, nurse-led complex care provider delivering high-quality care to children and adults in their own homes.
We are currently seeking a dedicated Care Coordinator to support individuals with complex physical disabilities and life-limiting conditions across Kent. This is an excellent opportunity for a passionate and organised professional to join a growing and supportive care team.
Key Responsibilities:
As a Care Coordinator, you will be responsible for setting up and managing care packages from referral through to delivery
Conduct care plans and risk assessments, ensuring all services meet clinical and regulatory standards
Liaise effectively with families, healthcare professionals, carers, and internal teams
Oversee the safe and timely mobilisation of new care packages
Maintain clear documentation and compliance with CQC regulations
What We’re Looking For:
Experience working as a Care Coordinator, ideally within a complex care or community setting
Strong communication, time-management, and multitasking skills
Understanding of health and social care regulations
A proactive and flexible approach to solving problems
