Cityworx are recruiting for a Care Coordinator in partnership with our client a leading complex care provider.
The role of Care Coordinator is a key role within the team, acting as the link between clients, healthcare professionals, and care staff. You’ll be responsible for managing schedules, coordinating care packages, and ensuring services are delivered safely, effectively, and with compassion. This role is fast-paced and detail-driven, requiring excellent communication, strong organisational skills, and a passion for providing exceptional care.
It is a full time office based position working from 12:00 hrs until 21:00 hrs Monday to Friday
Key Responsibilities
* Allocate care staff to home and community packages according to client needs.
* Build and manage staff rotas, ensuring full cover across all services.
* Maintain strong communication with clients and care teams via phone and email.
* Respond quickly to booking requests, emails, and calls.
* Use booking systems (Salesforce/Birdie) to maintain accurate records and documentation.
* Monitor and escalate safeguarding concerns or service issues promptly.
* Deliver excellent customer service and resolve issues effectively.
* Keep up-to-date with referrals, care plans, and internal policies.
* Collaborate with the Compliance Team to maintain a fully compliant workforce.
* Provide detailed handovers and updates to management.

* Proficiency in booking systems (preferably Salesforce or Influence).
* Discretion when handling confidential information.
