About the Role:
We are seeking a dedicated and organised Care Coordinator to join our domiciliary care team. You will play a vital role in ensuring the smooth and effective delivery of high-quality home care services. The ideal candidate will have a passion for supporting vulnerable adults and the ability to manage care schedules, communicate effectively with carers and service users, and uphold compliance with CQC standards.
Key Responsibilities:

Coordinate daily care visits for clients, ensuring rota coverage and continuity of care.

Liaise with care workers, service users, families, and external agencies to maintain high standards of service.
*
Ensure care plans and risk assessments are accurate, up-to-date, and regularly reviewed.
*
Respond promptly to any issues or changes in care needs, including emergency cover arrangements.
*
Support recruitment, onboarding, and training of new care staff as required.
*
Participate in the on-call rota, providing out-of-hours support as needed.
*
Ensure compliance with CQC regulations and company policies.
*
Maintain accurate records using care management systems.
*
Conduct spot checks, supervisions, and quality assurance visits in the community.
Requirements:
Essential:
*
Previous experience in a care coordination or domiciliary care role.
*
Excellent communication and organisational skills.
*
Strong understanding of scheduling and rostering systems.
*
Ability to work under pressure and meet deadlines.
*
Full UK driving licence and access to a vehicle.
*
Good working knowledge of CQC regulations and safeguarding procedures.
Desirable:

NVQ Level 3 in Health & Social Care (or working towards).
*
Experience with electronic care management software (e.g., PeoplePlanner, CarePlanner, PASS).
*
Local area knowledge is an advantage.
