Salary: £25,065 - £26,734 (Full-Time Equivalent) | Part Time Hourly Rate
Employment Type: Contract
Are you a motivated and organised individual looking to make a difference in the healthcare sector? We are currently seeking a Care Navigator to join our Clinical Services and Urgent Care teams in York. As a Care Navigator, you will play a pivotal role in ensuring patients receive high-quality care and a seamless experience.
Key Responsibilities:
Deliver high-quality administrative support, including appointment booking and referral processing
Manage patient data accurately and confidentially using SystmOne
Respond to patient and professional enquiries in a professional and compassionate manner
Support the coordination of care across services and follow up on patient needs
Work flexibly across teams to support service delivery
What We’re Looking For:
Level 2 English and Maths (or equivalent experience)
Excellent attention to detail and accuracy
Strong organisational and administrative skills
Excellent verbal and written communication
Confident using Microsoft Office applications
Ability to work independently and as part of a team
Experience in the NHS or general practice
Familiarity with SystmOne and referral systems such as Gateway
