Spencer Clarke Group are seeking a Case Manager for a local authority client in North London.
In this role, you will work collaboratively within a multidisciplinary team to deliver person-centred, strengths-based support that promotes independence, reduces delays, and ensures residents receive the right help at the right time.
Key Duties:
Work collaboratively within a multidisciplinary team to deliver timely, person-centred, and strengths-based support.
Promote independence by connecting individuals with informal and community-based resources wherever possible.
Contribute to integrated planning and decision-making through a shared point of access, reducing delays and duplication.
Build effective partnerships with residents, carers, professionals, and community organisations to achieve better outcomes.Qualifications and Experience:
The successful candidate will have the following skills / experience:
Experience working with adults in a strengths-based, person-centred way
Proven ability to work collaboratively in multidisciplinary teams
Knowledge of community resources and integrated care approaches
Confident in building relationships with residents, carers, and professionalsWhat's on offer:
Salary: £27ph
*may negotiate higher for exceptional candidates, based on experience*
Contract type: 3-6 months minimum, with a high likelihood of extension
Hours: Monday to Friday, 35 hours per week
