Working Hours: Mon – Fri, 09:00 – 17:00, 37 Hours per week

Responsibilities
Fulfill a social work role in a multi-disciplinary setting, collaborating across professional boundaries.
Assess and develop clear and effective care plans in partnership with clients, their families, and other interested parties.
Support clients and their carers during periods of major changes and stress, respecting diverse cultures, backgrounds, religions, and circumstances.
Develop positive working relationships with internal and external partners, such as Health, Commissioning, and Housing.
Person Specification
Experience in a social work role within a multi-disciplinary setting.
Ability to create effective care plans in collaboration with clients and stakeholders.
Competence in working with diverse populations, providing support during significant life changes.
Proven track record of building strong partnerships with various internal and external agencies.
