Job Description

Job Purpose: To provide practical and emotional support to children and young people with disabilities, mental health needs, or complex needs, enabling them to live as independently as possible in their homes and communities. The role supports the development of life skills, promotes wellbeing, and encourages social inclusion, in line with each child’s care plan or support plan. Key Responsibilities: * Provide one-to-one and group support to children and young people in the community or home setting. * Assist with daily living tasks such as personal care, meal preparation, medication prompts, and mobility. * Promote and encourage the development of independent living skills, such as budgeting, cooking, using public transport, and personal hygiene. * Support access to community activities, education, employment, and recreational opportunities. * Build positive, trusting relationships with children and young people, offering consistent and appropriate emotional support. * Work collaboratively with families, carers, schools, and other professionals (e.g., social workers, health workers). * Contribute to care planning, attend reviews, and maintain accurate records of support provided. * Monitor and report on the wellbeing, progress, and risks related to each young person. * Promote safeguarding and ensure the safety and welfare of all children and young people at all times. * Follow policies, procedures, and professional standards, including those related to equality, confidentiality, and data protection. Person Specification: Essential: * Experience working with children or young people with disabilities, complex needs, or in care settings. * Knowledge of child development and safeguarding practices. * Ability to work independently and as part of a team. * Strong communication and interpersonal skills. * Flexibility to work evenings, weekends, and occasional holidays as needed. * Full driving licence and access to a vehicle (if required for community visits). * Enhanced DBS check (or willingness to undergo one). Desirable: * Relevant qualification (e.g., NVQ Level 2 or 3 in Health and Social Care, Children and Young People’s Workforce). * Understanding of the challenges faced by young people transitioning to adulthood and independent living. Working Conditions: * Community-based role, with work carried out in service users’ homes and local settings. * Flexible shifts including evenings and weekends. * Emotional resilience and physical ability may be required due to the nature of the role
