Community Case Manager RNLD – Supported Living, Bristol
We are seeking an experienced Community Case Manager RNLD to join a respected provider of complex care for adults and children with learning disabilities and complex needs across Bristol and the surrounding areas. This is a full-time, permanent role, Monday to Friday (9–5), with hybrid working available – two days from home and three days in the community. The position offers a competitive salary of £50,000–£55,000 DOE.
As a Community Case Manager RNLD, you will take the lead in overseeing supported living services for individuals with learning disabilities, autism, and complex care needs. You will work closely with service users, families, and multidisciplinary teams to ensure care is person-centred, safe, and empowering, enabling individuals to live as independently as possible within their own homes.
Key Responsibilities
Manage and oversee supported living care packages for individuals with learning disabilities and complex needs.
Conduct holistic assessments and develop person-centred care plans tailored to each individual’s needs, preferences, and goals.
Support and mentor nurses and support staff to deliver outstanding care and promote independence for service users.
Act as the main point of contact for families, commissioners, CHC teams, and local authorities.
Lead on clinical governance, safeguarding, and quality improvement initiatives within supported living settings.
Ensure regulatory compliance and that all care is delivered in line with CQC standards.
What We’re Looking For
Registered Learning Disability Nurse (RNLD) with active NMC registration.
At least 2 years’ post-qualification experience, ideally within supported living or learning disability services.
Strong understanding of positive behaviour support and person-centred care planning.
Full UK driving licence and access to a vehicle; willing to travel across Bristol and surrounding areas.
