Job Description
Community Team in Slough looking to recruit a Discharge to Assess Occupational Therapist to join a supportive and fast-paced team. This is a fantastic opportunity to be part of a highly collaborative service focused on enabling timely and safe discharges from hospital. The role offers a full-time locum contract at £36.80 per hour, a fair and manageable caseload, and strong multi-disciplinary support.
Main Duties for the Discharge to Assess Occupational Therapist include:
Supporting the Discharge to Assess pathway by facilitating efficient hospital discharges.
Working closely with the Hospital Social Work Team to assess patient needs in the community.
Delivering timely occupational therapy assessments and interventions to promote independence at home.
Managing a caseload within a community setting as part of a team of three experienced OTs.
Ensuring service users and their families are fully engaged in the discharge planning process.
Applying sound clinical reasoning and maintaining accurate, up-to-date documentation.
To Be Successful:
Previous experience working as an Occupational Therapist in a hospital discharge or community rehab setting.
Ability to work autonomously and hit the ground running.
HCPC Registration.
Enhanced DBS/CRB (we can obtain this for you).
Eligibility to work in the UK.
Recognised Occupational Therapy qualification.
