Job Description

Early Help Practitioner Location: 2-4 Malton Road, W10 5UP Start Date: ASAP Contract Duration: 2+ Months Working Hours: Mon – Fri, 09:00 – 17:00, 37 Hours per week Pay Rate: £ 20.86 per hour  Job Ref: (phone number removed) Responsibilities Provide early intervention support to children, young people, and families to improve outcomes and prevent the escalation of issues. Work collaboratively with schools, local authorities, and other agencies to deliver tailored support plans for children and families. Conduct assessments to identify the needs of children and families, ensuring that appropriate services are accessed. Maintain accurate and up-to-date records of all interactions and interventions in line with confidentiality and data protection policies. Participate in multi-agency meetings and contribute to the development of integrated support strategies. Provide guidance and support to families to help them access resources and services within the community. Person Specification Qualified in social and healthcare fields with relevant certifications and registrations. Proven experience in working with school-age children and young people, ideally within an agency setting. Strong IT literacy skills, with the ability to efficiently manage digital records and communication. Excellent communication and interpersonal skills to effectively engage with children, families, and professionals. Ability to work independently and as part of a team, managing a varied caseload with competing priorities. Demonstrated commitment to equal opportunities and diversity in practice.
