Family Outreach Worker Oldham (hybrid working with office, community and home visits) £27,013.56 - £29,552.02 per annum Full-time, Permanent Job Description: Are you passionate about supporting families and individuals to overcome challenges and achieve positive change? We are recruiting for a Family Outreach Worker to join a dedicated early intervention and prevention service in Oldham. This rewarding role focuses on supporting families, adults and households before issues escalate, helping them access the right support and build the confidence, resilience and skills needed to achieve long-term positive outcomes. Working across the community, in people's homes and from the office, you will manage a varied caseload, carry out assessments and deliver tailored support plans. You'll work closely with local services and partner agencies to empower individuals and families to make sustainable changes and improve their wellbeing. The role involves: Managing a caseload of families, adults and households requiring early intervention support Completing assessments and developing personalised support plans tailored to individual needs Providing one-to-one outreach support, guidance and practical assistance Building positive and trusting relationships with service users and their families Supporting individuals to access services such as housing, debt advice, mental health support and parenting programmes Working collaboratively with partner agencies and professionals to achieve positive outcomes Maintaining accurate records using electronic case management systems Monitoring progress and reviewing support plans to ensure sustainable long-term change Undertaking community-based and home visits across Oldham Promoting independence, resilience and self-confidence through strengths-based approachesYou’ll need: Extensive experience completing early intervention assessments and developing effective support plans Experience managing a caseload within a support, community, housing, social care or related setting Strong relationship-building skills with the ability to engage families, adults and young people Excellent communication and interpersonal skills The ability to work independently and as part of a multi-agency team A good understanding of safeguarding principles and professional boundaries Strong organisational skills with the ability to manage competing priorities A passion for helping people make positive and lasting changes in their lives Flexibility to work across office, community and home-based settings If this sounds like the perfect role for you then please submit your CV.  You will be required to undertake an enhanced DBS check if you are successfully offered this role.
