Job Description

Family Wellbeing Case Practitioner Location: Civic Centre, SM4 5DX Start Date: ASAP Contract Duration: 3+ Months Working Hours: Mon – Fri, 09:00 – 17:00, 37 Hours per week Pay Rate: £ 20.85 per hour  Job Ref: (phone number removed) Responsibilities Gather and analyze information to assess vulnerable children, young people, and their families, including those with diverse and complex needs. Formulate and implement family plans based on SMART objectives to inform ongoing interventions. Coordinate services for children and families by working in partnership with professionals from a range of multi-disciplines to bring about positive change. Engage and empower individuals to bring about change and achieve positive outcomes for children, young people, and families with multiple and complex needs and risk factors. Person Specification Understanding of Child and Adolescent Safeguarding and child protection policy. Experience in working with children, young people, and families with a range of needs and risk factors. Knowledge of policies, statutory regulations, and guidance related to child development, safeguarding, child protection, SEND, youth crime, and antisocial behavior. Ability to work appropriately within policies and regulations in practice.
