Role Summary:
The Support Worker provides person-centred care and practical support to adults with learning disabilities and/or acquired brain injuries in a supported living environment. The goal is to empower clients to live as independently as possible while maintaining their physical, emotional, and social wellbeing.
Key Responsibilities:
1.
Person-Centred Support:

Support clients based on their individual care plans, needs, preferences, and goals.
*
Promote independence by encouraging clients to participate in daily activities, decision-making, and community life.
2.
Daily Living Support:
*
Assist with personal care (e.g., washing, dressing, toileting) where required.
*
Support with domestic tasks such as cooking, cleaning, and shopping.
*
Help manage medication, appointments, and healthcare routines.
3.
Communication and Relationship Building:
*
Build trusting, respectful relationships with clients.
*
Use alternative communication methods when required (e.g., Makaton, pictorial aids).
*
Maintain professional and compassionate communication with families and other stakeholders.
4.
Behavioural and Emotional Support:
*
Understand and respond appropriately to challenging behaviour, using positive behaviour support (PBS) techniques.
*
Provide emotional reassurance, encouragement, and crisis de-escalation when needed.
5.
Health and Safety:
*
Monitor and report any changes in clients’ physical or mental health.
*
Ensure the living environment is safe, clean, and adapted to client needs.
*
Follow safeguarding, confidentiality, and risk management policies.
6.
Record Keeping:
*
Accurately complete daily logs, incident reports, and care documentation.
