Job Description

Health and Wellbeing Practitioner- HMP Send (part time) Location: Surrey Salary: £13,928.57 per annum Vacancy Type: Permanent, Part Time About The Role The Forward Trust is seeking a compassionate and motivated Health & Wellbeing Practitioner to join our team at HMP Send, Surrey, working 18.75 hours per week. As part of our integrated Health & Wellbeing Service, you'll support women experiencing substance misuse and low-level mental health challenges, helping them build healthier, more positive futures within a trauma-informed and gender-responsive environment. Working as part of a multi-disciplinary team, you'll deliver recovery-focused interventions, provide practical support, and help service users prepare for successful reintegration into the community. What you'll be doing: * Managing a caseload and coordinating individual support plans * Conducting assessments and developing person-centred care plans * Delivering structured one-to-one sessions and facilitating group programmes * Providing harm reduction advice and guidance * Supporting release planning and through-care arrangements * Working closely with healthcare, mental health, and prison colleagues * Maintaining accurate records and contributing to service performance targets * Undertaking second signatory duties and oral swab testing where required What we're looking for: * Experience supporting individuals with substance misuse, mental health, or complex needs * Excellent communication and relationship-building skills * A proactive, organised, and solution-focused approach * Confidence working within a multi-disciplinary environment * Strong IT and record-keeping skills * Commitment to completing the Drug and Alcohol Workforce Development Programme (DAWFDP) You'll be part of a dedicated team transforming lives every day, with access to ongoing training, professional development, and the opportunity to make a lasting impact in a unique and rewarding setting.
