Health Care Assistant (End of Life Care)

Role Overview
We are seeking compassionate and dedicated Care Assistants to adults in the community. You will be responsible for delivering high-quality care that promotes independence, dignity, and well-being of people by providing responsive care in their homes including people nearing end of their life. This role involves assisting clients with daily living activities, personal care, medication support, and companionship.
Key Responsibilities
* Provide personal care, including washing, dressing, and toileting, while maintaining dignity and respect. Assist with safe repositioning and use of mobility equipment (profiling beds, slide sheets).
* Promote and protect the dignity, privacy, and preferences of those in care. Assist with meal preparation, feeding, and promoting healthy nutrition.
* Support with medication administration in line with care plans. Help with household tasks such as cleaning, laundry, and shopping.
* Offer companionship, dignity-focused support and emotional support, encouraging social interaction.
* Apply skin care protocols and maintain skin integrity. Work as a team for scheduled visits.
* Support individuals with mobility needs, using appropriate equipment as required. Monitor and report changes in clients’ conditions and comfort levels to relevant healthcare professionals.
* Ensure compliance with care plans, safeguarding policies, and health & safety regulations. Provide waking night support (as needed), staying vigilant throughout.
* Support families sensitively through anticipatory grief and post-death processes (e.g., notifying DN or GP). Accurately record observations, actions and concerns via digital or paper logs
Requirements
* Experience in domiciliary care, community care, supported living, mental health, learning disabilities, or behaviours that challenge is desirable.Knowledge of anticipatory medicines, symptom observation, and end-of-life pathways.
* Previous experience in End-of-Life or Palliative Care A caring, patient, and empathetic nature.
* Excellent communication and interpersonal skills. Ability to work independently and as part of a team.
* A flexible approach to work, including availability for evenings and weekends. Enhanced DBS
* A valid UK driving licence and access to a vehicle is critical. Manual handling training and experience with equipment
Desirable:
* NVQ Level 2 or 3 in Health and Social Care (or working towards).
* Experience supporting individuals with stomas, catheters or NG feeding.
* Experience of working with people with fluctuating capacity and on DoLS orders.
