What We're Looking For:
A minimum of 3 years' post-qualified experience as a social worker, ideally with experience in a hospital, health, or adult social care setting.
Current and valid Social Work England registration.
A strong understanding of relevant legislation (e.g., Care Act 2014, Mental Capacity Act 2005) and experience in discharge planning, safeguarding, and complex case management.
Excellent communication and assessment skills, with the ability to work effectively with patients, families, and multi-disciplinary healthcare professionals under pressure.
