Job Description
Vale of Glamorgan Adult Services are looking for a Social Worker to be a part of their busy Intake and Assessment team.
* Completing needs assessments in people's home
* Completing MCA's / BIA's
* Typing up assessments, Care and Support plans, and submitting Service Requests
* Facilitating Respite and long-term placements in care homes
* Being flexible to work on duty if required
* Attend the Civic Offices in Barry at least twice a week

* A relevant Social Work Qualification
* Registration with Social Care Wales
* Enhanced DBS registered on the update service or be happy to apply for a DBS
* 3 Years of referencing
* Driving license with access to a car
