We are looking for a dedicated and compassionate CHC Nurse to join a local ICB 3-month contract. The CHC Nurse will ensure that all care and support needs are assessed and reviewed with clinical accuracy, compassion, and respect, using the Decision Support Tool (DST) to uphold best practice and legal compliance throughout the Continuing Healthcare process.
Key Responsibilities:
Conduct face-to-face CHC assessments and reviews.
Cluster assessments over a few days to optimise travel and time.
Work independently while liaising with MDTs and families.
Ensure documentation aligns with the National CHC Framework.Requirements:
Registered Nurse or Allied Health Professional.
Strong CHC experience, especially with FNC and DSTs.
