About the Role:
We are seeking an experienced Continuing Healthcare (CHC) Nurse Assessor for a 3-month interim contract in the North West. This full-time role requires 5 days per week, fully face-to-face, conducting assessments, reviews, and working closely with patients, families, and MDTs across the region.
Key Responsibilities:
Undertake face-to-face CHC assessments and reviews in line with the National Framework
Produce clear and concise DSTs with sound clinical rationale
Engage with multi-disciplinary teams, patients, and families
Ensure timely documentation and adherence to CHC timelines
Work independently while maintaining high professional standards
