Job Summary:
Eden Brown Synergy is currently supporting a leading NHS organisation in Central London seeking two experienced Practice Nurses to join their Primary Care Team across Hammersmith and Charing Cross GP sites.
Duties and Responsibilities:
Deliver high-quality nursing care within a primary care setting
Conduct chronic disease and long-term condition reviews (e.g. diabetes, asthma, COPD, hypertension)
Administer immunisations and vaccinations for all age groups
Perform cervical screening and support women's health clinics
Participate in clinical audits, infection control, and health promotion initiatives
Maintain accurate patient records using SystmOne (experience desirable)
Work collaboratively with GPs, Clinical Pharmacists, and the wider MDT
Requirements:
Registered Nurse (NMC PIN) with Practice Nurse qualification
Experience in a GP or primary care environment
Confident in chronic disease management and vaccination protocols
Cervical smear training desirable
Strong communication and documentation skills
