Working with residents admitted for mental health needs

Preventing unnecessary admissions & facilitating timely discharges
*
Collaborating with acute, rehab, forensic & general wards

Conducting Care Act assessments & strength-based support plans

Partnering with health, housing, & voluntary sector agencies
*
Completing carers assessments & empowering caregivers
*
Performing mental capacity assessments & best interests meetings
*
Carrying out reviews to ensure support is effective and proportionate

Strong case management, risk assessment & multi-agency collaboration

Deep understanding of adult social care legal frameworks
*
Minimum 1 year post-ASYE statutory experience
