Cityworx is proud to be recruiting for a Care Package Mobilisation Coordinator for an established complex care company within the Leeds area. Please find the details below:
Job Summary
You will be responsible for supporting the smooth and timely onboarding of new care packages. This includes coordinating the practical and logistical steps needed to start each package safely and in line with client needs, compliance requirements, and company standards. You will work closely with internal teams, clients and care staff, to ensure the initial phase of care is set up effectively and then handed over to the long-term team.
Key Responsibilities
* Coordinate the onboarding of new care packages under the guidance of senior management.
* Liaise with clients, families, and commissioners to gather information required for mobilisation.
* Schedule and arrange initial client assessments in collaboration with the Clinical Team.
* Work with the Recruitment and Coordination teams to ensure appropriately trained staff are assigned to new packages.
* Prepare and send staff profiles to clients/families for approval and arrange introductions or meet-and-greets.
* Produce and finalise the initial rota for each new care package and gain sign-off from the client/family.
* Maintain detailed and accurate mobilisation records using company systems.
* Ensure compliance with internal policies and CǪC standards during the mobilisation phase.
* Support the handover process to the ongoing care team once mobilisation is complete.
* Escalate any delays, concerns, or challenges in mobilisation to the relevant manager in a timely manner.
* Attend internal meetings relating to mobilisation planning and provide updates on progress.
* Contribute to a culture of responsive, safe, and high-quality care.
Skills and Attributes
* Strong organisational and time management skills.
* Excellent communication and interpersonal abilities.
* Confident working with families, clinicians, and commissioners.
* Ability to work independently, manage priorities, and meet deadlines.
* Attention to detail and a methodical approach.
* Good IT skills including Microsoft Word, Excel, Outlook, and rostering platforms.
* A flexible, can-do attitude with a willingness to travel when required.
Experience and Ǫualifications
Essential
* Minimum 1 years’ experience in health or social care.
* Experience in a support, coordination, or administration role.
* Understanding of complex care services.
* Comfortable communicating with clients and professionals.
Desirable
* Experience supporting care package mobilisation.
* NVǪ Level 2 in Health C Social Care or equivalent.
* Understanding of CǪC requirements in care delivery.
* Familiarity with electronic rostering or care management systems
