Key Responsibilities of a Mobilisation Nurse:
Carry out holistic assessments and develop detailed mobilisation plans.
Oversee onboarding, carer training, and competency reviews.
Provide visible support during the first four weeks of each new package.
Collaborate with families, CHC teams, case managers, and internal staff.
Represent the provider at multidisciplinary meetings to support service growth.

Requirements for the Mobilisation Nurse role:
NMC Registered Nurse (Adult or Children’s)
Background in community, complex, or continuing care
Strong communication, planning, and coordination skills
Flexibility to travel across North London and work outside standard hours
Full UK driving licence and access to a vehicle
