As a Nurse Case Manager, your responsibilities will include:
Overseeing the implementation and management of complex care packages for children.
Conducting new client assessments and leading team meetings.
Performing medication and clinical audits for compliance.
Providing clinical supervision and competency sign-off for nurses.
Liaising with the multidisciplinary team for coordinated care.
Offering standard and emergency support to regional care functions.
Supporting service delivery in line with person-centred care plans.
Ensuring care staff are trained in personal care and specialist equipment use.

Standard working hours of Monday to Friday, 9-5
The ideal candidate for the Nurse Case Manager role will have:
Registered Nurse (Paediatrics) or Midwife with valid NMC registration.
Extensive experience in paediatric complex care including tracheostomy, ventilation, spinal injuries, and brain injuries.
Community experience working with both paediatrics and adults.
