We are seeking an experienced and motivated Occupational Therapist to join the Community Occupational Therapy Team within Porth Ceredigion. This is an exciting opportunity to work within an innovative Through Age and Wellbeing Integrated Services Model, helping adults and older people maximise independence, safety, and quality of life through high-quality, person-centred occupational therapy.
About the Role
As a highly skilled Occupational Therapist, you will autonomously manage a varied and often complex caseload across the community. You will use your specialist knowledge to assess needs, recommend assistive equipment and housing adaptations, and deliver evidence-based interventions that empower individuals to live as independently as possible.
Working closely with colleagues across health, social care, housing, and partner organisations, you will play a key role in delivering integrated, preventative, and strengths-based support.
Key Responsibilities
* Manage complex occupational therapy assessments and interventions within the community
* Provide specialist advice on equipment, adaptations, moving and handling, and safe environments
* Develop person-centred intervention plans with individuals, families, and carers
* Undertake manual handling risk assessments and care planning
* Supervise and delegate work to Occupational Therapy Assistants as required
* Work collaboratively within a multi-disciplinary and multi-agency environment
* Maintain professional standards in line with HCPC and RCOT guidance
* Support students, colleagues, and service development initiatives
What We’re Looking For
You will have:
* A Degree or Diploma in Occupational Therapy
* Current HCPC registration
* Post-graduate experience in community occupational therapy
* Experience managing complex caseloads and working in multidisciplinary teams
* Strong clinical reasoning, communication, and problem-solving skills
* Knowledge of relevant Welsh legislation and safeguarding frameworks
* A flexible, compassionate, and person-centred approach
* A full driving licence and ability to travel across the community
