This is an exciting opportunity to join our dynamic Hackney Occupational
Therapy and Sensory Service within our Hospital Team.
This role is both dynamic and challenging, placing the customer at the centre
of service delivery. You will provide outstanding occupational therapy
interventions that promote independence in daily living activities, fostering an
ethos of co-production to give residents choice and control.
The Integrated Discharge Service assesses Hackney residents, whether in
hospital across London and beyond, to identify the most suitable and safe
discharge destination and support plan. We prioritise safe discharges that
respect individualsʼ legal rights and wishes. The team collaborates with
multi-disciplinary hospital teams and attends their meetings on relevant
wards. The service is inclusive of those in acute settings with a learning
disability, mental health diagnosis, or dementia. Also, it supports residents
with no recourse to public funds who have eligible care needs. It operates
seven days a week, and staff are welcome to contribute to weekend working if
they wish. Community Reviews are carried out collaboratively with residents,
family members, carers, and other professionals to ensure long-term needs
are assessed and any emerging concerns addressed.

Required Skills and Qualifications
● Clinical Experience: Experience working with adults in a healthcare
setting, ideally within a hospital or social care environment.
● Assessment Skills: Strong analytical and assessment skills to identify
functional limitations and needs.
● Ability to complete a Care Act Review, as knowledge of Care Act 2014
legislation is paramount.
● Knowledge of safeguarding principles, MCA, etc.
● Communication Skills: Excellent verbal and written communication skills
to interact effectively with patients, families, and colleagues.
● Problem-Solving: Ability to creatively problem-solve and adapt
interventions to individual patient needs.
● Empathy and Compassion: A compassionate and empathetic approach
to patient care.
● Teamwork: Ability to work effectively as part of a multidisciplinary team.
● Organisational Skills: Good organisational and time management skills
to manage a caseload effectively.
This is a hybrid opportunity; the expectation is to work twice a week in the office.
