Service Care Solutions are recruiting for an Outreach Worker to join a dedicated team in Northampton. This is a full-time role for approximately 3 months, with the possibility of extension.
Key Responsibilities:
Engage with individuals experiencing street-based living, including rough sleepers and beggars, to support them into more stable lifestyles.
Empower service users through increased choice and involvement, working collaboratively to achieve sustainable change and positive outcomes.
Build on service users’ personal strengths and social networks, both in the UK and in their country of origin.
Deliver flexible and effective interventions to engage hard-to-reach individuals.
Conduct outreach shifts at various times and locations to rapidly assist rough sleepers.
Provide assertive outreach to entrenched rough sleepers, including those initially unwilling to engage.
Support timely hospital discharges by securing appropriate accommodation for homeless service users.
Collaborate with drug and alcohol teams to ensure access to treatment for substance misusers.
Offer advice using recognised models such as Motivational Interviewing to help service users explore and address substance misuse.
Work with mental health agencies to ensure access to appropriate support.
Facilitate ongoing healthcare access for service users.
Experience of working as part of an outreach team
Experience of working with rough sleepers
Experience of working in a social care environment
Experience of working within the substance misuse sector
