Job Title: Part-Time Care Coordinator
Location: South East London (SE13)
Hours: Part-Time, 24 hours per week (3 days)
Working Days : Mondays, Fridays, and 1 day every weekend (2 Saturdays & 2 Sundays per month)
Shift :10:00 AM - 6:00 PM
Salary : £15,238.08 per annum / £12.21 per hour
Hybrid Working : 2 days in the office, 1 day working from home
About the Role:
LEO Recruits is seeking a dedicated and organized Part-Time Care Coordinator to join our client's domiciliary care agency based in South East London (SE13). This is an excellent opportunity to contribute to the delivery of high-quality care services within the local community. The role is part-time, working 24 hours per week across three days, with a hybrid working model offering flexibility.
Key Responsibilities:
1. **Care Planning:** Develop, implement, and review personalized care plans with clients, families, and healthcare professionals.
2. **Client Assessments:** Conduct initial assessments, risk assessments, and ongoing evaluations to identify client needs.
3. **Care Scheduling:** Coordinate and schedule care visits, ensuring timely and efficient service delivery.
4. **Career Allocation:** Match care workers with clients, considering compatibility, skills, and availability.
5. **Communication:** Liaise with clients, families, care workers, and healthcare professionals to ensure seamless care delivery.
6. **Record Keeping:** Maintain accurate, up-to-date records, including care plans, visit logs, and client information.
7. **Quality Monitoring:** Monitor care quality, addressing concerns and implementing improvements.
8. **Compliance:** Ensure adherence to regulatory requirements, company policies, and procedures.
9. **Team Support:** Assist with care worker training, supervision, and performance management.
10. **Emergency Response:** Participate in on-call Rota (if required) and respond to care-related emergencies.
Essential Skills and Qualifications:
1. **Qualifications:** Diploma/NVQ Level 2/3 in Health and Social Care or equivalent.
2. **Experience:** Minimum 1 year in domiciliary care coordination or related field.
3. **Communication:** Excellent communication, interpersonal, and organizational skills.
4. **IT:** Proficient in Microsoft Office, care management software, and scheduling systems.
5. **Problem-Solving:** Strong problem-solving and analytical skills.
6. **Time Management:** Ability to priorities tasks and manage time effectively.
7. **Knowledge:** Understanding of relevant legislation, regulations, and care standards.
Desirable Skills and Qualifications:
1. **Additional Qualifications:** Qualifications in care management, nursing, or social work.
2. **Specialized Training:** Training in dementia care, palliative care, or other specialized areas.
3. **Language Skills:** Proficiency in additional languages.
4. **Experience:** Experience working with vulnerable adult groups (e.g., learning disabilities).
Working Conditions:
1. **On-Call:** Participation in on-call Rota may be required.
2. **Travel:** Occasional travel to client locations, meetings, and training events.
